1) Follow the instructions listed here https://lewisvillesports.rankonesport.com/AthleticPortal/OnlineForms/Form.aspx?Type=4

2) Click ELECTRONIC PARTICIPATION FORMS on the right side then individually select each form & fill out each of the 7 forms completely.

INCOMING FRESHMAN & JUNIORS ONLY

In addition to the electronic forms, all incoming Freshman & Juniors must complete the imPACT Concussion Baseline Test.

This test takes approximately 45 minutes to complete & must be done online using a wired mouse.  Please choose a quiet environment, with no distractions from TV, cell phones, pets, siblings, etc, while taking the test.  This must be completed by August 1st.

Instructions 

1) Go to www.impacttestonline.com/benhogan

2) Enter customer code JMGG46676W

3) Select "Hebron High School" then 'Launch Baseline Test'

4) Read the test instructions.  Instructions are not timed, but the test modules are.

5) Save test confirmation page for your records.

Reminder - Physical forms are also due to coaches by August 1st
