- Physical - on official UIL form, both pages or sides filled out, all signatures completed (doctor and parent)

- Medical History Form - please just submit once, it then goes to the trainers for review, so it will say incomplete, but as long as you have submitted it, they have it. Submitting more than once does not help, it’s just more work for the trainers to sort through.

VOLUNTEER OPPORTUNITIES
Parent Volunteers Needed – Hebron Volleyball 2026–2027 Season
Dear Hebron Volleyball Families,
Your involvement is vital to the success of our program.
For each home game, we need to fill approximately 24 volunteer positions across our 4 teams. These roles are essential for smooth game-day operations, and we truly can’t do it without your support.
We must fill the following roles for every home game:
Scorekeeper – Book
Libero Tracker
Scoreboard Operator
Concessions – Arena & Main Gym
✅ We are asking each family to volunteer at least 7 times during the season.
Please be dependable! If you sign up for a role and later find you cannot fulfill it, you are responsible for finding a replacement and updating the schedule accordingly.

️ BONUS: Volunteers receive free admission on the evening of your shift — no need to purchase a ticket!
